Dr Jack Rzepka has practiced Internal Medicine in Cleveland, Ohio for 21 years prior to moving to Arizona. He has been in practice in Arizona for two years serving with a collaboration of specialists. He is highly regarded by his patients and colleagues due to the level of care he provides. He takes pride in practicing in a knowledgable, caring, but compassionate manner. He promotes the physical and mental well being of each individual patient.

Shuree K. Oldehoeft, PA-C, is the founder. She has served patients for over ten years, first serving as an Emergency Medical Technician, then as a Board Certified Physician Assistant. She completed her Physician Assistant Studies at Union College in Lincoln, Nebraska. She is an active member of the National Academy of Physician Assistants, AZ Academy of Physician Assistants, American Academy of Home Care Providers, and the American Family Practice Physician Assistant Organization. She is also an active member of the Scottsdale and Cave Creek/Carefree Chambers.
